ILLINOIS TECH ATHLETICS PRE-PARTICIPATION PAGE 

Below is a list of forms and links to other documents that are a part of both Illinois Institute of Technology’s and the National Collegiate Athletic Association’s compliance process. All Student-Athletes must complete forms in the links below (pertaining to their standing) in order to start practice with their respective teams.   

Student-Athletes must complete and submit all NCAA and Athletic Department paperwork upon reporting to campus. Please follow the instructions next to each link, as different forms of completion are required for each document. Forms requiring print outs are to be delivered upon arrival on campus.

ALL STUDENT-ATHLETES MUST COMPLETE:

NEW STUDENT-ATHLETES MUST ALSO COMPLETE:

As of the 2024-2025 school year, ALL student-athletes, whether new or returning, need to get a new physical. 
Physicals must be dated June 1, 2025 or later. You must use the attached physical form with your physician. If you are an international incoming player, you can submit a physical from your home country, but it must be reviewed and approved by the Head Athletic Trainer and team physician. If it does not contain all required information, you may be asked to get another physical exam completed in the US.

RETURNING STUDENT-ATHLETES MUST ALSO COMPLETE:
Returning athletes, for the 2025-2026 school year please complete the transition from ATS to Healthy Roster. Follow the instructions linked below.
