All Illinois Tech student-athletes MUSTÂ complete all Required Pre-Participation Forms before competing in any intercollegiate sport. This includes completing a physical exam on the Illinois Tech physical form performed and signed by a physician. Also included is the completion of creating/updating your student-athlete portal and pre-participation forms on ATS.

Visiting Team Information
Visiting teams are welcome to utilize the athletic training room two hours prior to competition. The Athletic Training room is located in Keating Hall lower level at 3040 South Wabash Avenue, Chicago, Ill. 60616. The Illinois Tech Athletic training room has the following modalities available:
- 4 Channel EMS/US Machines
- Hydrocollator
- Treatment Tables
- Taping Area
Athletic trainers accompanying their team are welcome to use all modalities available. For teams traveling without an athletic trainer please send a detailed treatment letter prior to the teams arrival. If athletes wish to be taped by the Illinois Tech Athletic Trainers, please provide your own tape. On the sideline or bench, teams will find water, cups, and injury ice. The host athletic training staff will have an AED, splint bag, and crash bag.
